A lackadaisical attitude among crew members on the USS Hartford submarine led to a collision with the USS New Orleans in the Straits of Hormuz in March 2009, causing damage of over $104 million, according to a heavily-redacted copy of a naval investigation obtained by The Day, a Connecticut newspaper. The report noted that had standard procedures been followed at any of thirty opportunities, the collision could have been averted.
